Step 1 - Contact Aging Assistant for an Initial Consultation
During this initial conversation, we can answer your questions about Home Care Services and briefly assess your needs. Following this brief assessment, we can identify a Care Manager for you and your family. We would then move to Step Two.
Step 2 - Conduct Full Assessment
Your Care Manager will meet with you for a free in-home consultation. During this initial meeting with you and your family, the Care Manager assesses the client’s individual needs and records physical, lifestyle and personal preferences. We might ask questions pertaining to the clients needs such as:
* Are you taking any medications?
* How are those medications managed?
* What do you like for breakfast?
* Do you prefer a bath or a shower
Based on the information gathered during this assessment we will provide you and your family with your pricing options.
Step 3 -Develop Detailed Care Plan
Your Care Manager prepares and reviews a comprehensive home care service plan which outlines specific instructions for:
* Personal care: bathing, eating, dressing, toileting
* Household care: cooking, cleaning, laundry, shopping
* Health care: medication management, physicians’ appointments
* Emotional care: companionship, meaningful activities, conversation
Step 4 -Select Caregiver and Begin Orientation
Once you agree to the recommendations to the care plan, we recommend the Caregiver who has the most appropriate experience, skills and personality for your situation. You and your family meet the Caregiver , and if you feel comfortable, the home care service begins.
The Care Manager provides an orientation for the Caregiver , where we communicate all the information supplied during the consultation.
Step 5 -On Going Quality Control and Care Revisions
Your Care Manager contacts you and your family routinely to discuss your Caregiver services. Additionally, our Supervisors will perform periodic visits to the home to assess your Caregiver , your level of care and determine if changes are needed. Your satisfaction with our services is our commitment and priority.
